Aetiology
- Ischaemia - most comnon
- Toxins (contrast, gentamicin, myoglobin)
- Acute tubulointerstitial nephritis
- Urinary obstruction
Pathogenesis
- Initiation: Ischaemia/ damage to tubular cells
- Maintenance: Nadir. Worsened by tubuloglomerular feedback (inc salt due incompetency of more proximal tubules --> vasoconstrict afferent arteriole), local vasoconstrictive cytokines
- Recovery: parenchymal repair and regeneration
