NYHA dyspnoea:
- Class 1: Extremes of activity
- Class 2: Greater than ADL
- Class 3: ADL
- Class 4: @ rest
Epidemiology
Highest mortality in DEM
Management
- DRSABCDE
- Titrate SaO2 to about 98%. 100% too much (N = 90-98%)
- Sats <90% (~60mmHg PaO2) on Max O2 (15L/min) = MET call
- Cyanosis unreliable: happens with SpO2 = 58% ~ PaO2 30mmHg
- Problem with O2 = interstitial/ perfusion problem
- Problem with CO2 = ventilation problem
Additional knowledge
- O2 receptor cells beside carotid bodies
- Exogenous O2 RAISE PaCO2 (O2 --> inc resp drive (depth - flow rate) --> more air (90L/min) than O2 (15L/min) --> inc PaCO2
- O2 Sat = # of Hb with O2/ # of Hb
- O2-Hb dissociation curve: Sats = reserve, PaO2 = driver of O2 movement
- O2 less soluble in water than CO2
- PAO2 ~ PaO2 in health
