Symptoms
Acute cholecystitis
- Severe constant (hours to days) epigastric/ RUQ pain, radiating to R shoulder
- n+v
- Fever
- Flatulent dyspepsia
- Vague abdo discomfort
- Fat intolerance
Risk Factors
Acute: 5 Fs (refer to gallstones)
Signs
- Murphy's sign/ Sonographic Murphy sign
- Local peritonism (rigidity)
- Palpable tender GB
Blood: FBC+CRP, LFT (very high GGT, high ALP)
Imaging: US
Management
- Admit, hydrate, analgesia, NBM
- Abx (Cipro + Metronidazole)
- Cholecystectomy
